Code cleanup
The code that parses output from the different players has been cleaned up. ESD support has been removed. STDOUT has been added as an output device. This is suitable for piping into e.g. ices for streaming. The .stm extension has been added to the list handled by ModPlayer.

Minor feature enhancements
"Play status" now looks a lot better and doesn't waste as much screen space. There are various robustness changes. More advanced controls have been added. Tables have been made read-only. Double-click functionality has been added to the music list and the queues. The song title is displayed in the title bar.
